Feeding

Baby is feeding at the breast or bottle without distress
Weight trend understood (any weight loss recovered or being monitored)
You know approximately how many feeds and wet/dirty nappies to expect in 24 hours
If formula feeding — you know safe preparation and storage

Newborn examination completed by your paediatrician (the discharge check happens during your stay, by the day before you go home)
Newborn Bloodspot Screening Test (heel-prick) done, or arranged if leaving before 48 hours
Hearing screen done (or outpatient screen arranged)
Vitamin K given (with consent)
Hepatitis B vaccine (birth dose) given if consented — recorded on immunisation history

Safe sleep set-up at home — back to sleep, own cot or bassinet in your room, firm flat surface, face uncovered, smoke-free, no loose bedding, bumpers or soft toys
Car seat fitted and checked by an authorised restraint fitter
You know how to bath baby safely
You know how to contact your Maternal & Child Health Nurse
MCH nurse home visit or first appointment booked
Follow-up arrangements understood — MCH visit in the first week, paediatric review at 6 weeks, plus anything extra in your discharge plan

Baby’s Medicare enrolment form submitted
Birth registration form started
Paediatric follow-up appointment booked — email or call the rooms early, appointments can book out weeks in advance
NeoPaeds registration form completed (forms.neopaeds.au/register) — if you submitted it after your last in-hospital review and it notes something needing discussion, please book a consultation to go through it
