A few things to know
- Receiving care from the practice means you’re agreeing to our practice terms — our fees & billing, privacy policy and medical disclaimer are published on this site.
- The earlier the form is in, the better: a promptly completed form gives us the chance to talk through anything it raises — family history, feeding, your individual circumstances — while you’re still in hospital.
- If the form arrives after the final in-hospital review, anything outstanding in it will need a separate follow-up consultation after discharge.
